You can pick up the form at:

• Location: Ministry of Health, Block 8, Room 69, Idongesit Nkanga Secretariat, Uyo
• Start Date: Wednesday, 11 June 2025
• Time: 9:00am to 3:00pm (weekdays only)
• Fee: ₦15,000 (non-refundable)

They don’t sell forms on weekends or public holidays, so plan your visit carefully.

• Environmental Health Technician – 3 years
• ND in Environmental Health Technology – 2 years
• HND in Environmental Health Technology – 2 years
• Pharmacy Technician – 3 years
• Medical Laboratory Technician – 3 years
• Professional Diploma in Health Information Management – 3 years
• Community Health Junior Extension Worker – 2 years
• Community Health Extension Worker – 3 years
• HND in Public Health Nursing – 2 years

Entry Requirements

You must have:

• Five credits in WAEC, NECO, or NABTEB
• Subjects must include English, Mathematics, Biology, Chemistry, and Physics
• Other relevant subjects like Geography, Agric Science, or Technical Drawing might be accepted

For Medical Lab Technician and Environmental Health Technician, NABTEB is strongly preferred.
