• Academic Certificates: Including FSLC, WASSCE, SSCE, GCE, NECO, and NABTEB with at least five credits in English, Mathematics, Chemistry, Physics, and Biology. Note: Only a maximum of two sittings are accepted, except for NABTEB which cannot be combined with other results.
• Professional Testimonial: From the last professional school attended.
• Proof of Identity: Birth Certificate or Age Declaration
• Practicing License: Current and valid.
• Change of Name Certificate: If applicable.
• State of Origin Certificate.
• Passport Photographs: Two recent ones.
