CONSA Post UTME Form for ND Nursing Programme | College of Nursing Sciences, Adazi-Nnukwu

The College of Nursing Sciences, Adazi-Nnukwu (CONSA) Post UTME Form is now available for the 2024/2025 academic session.

Applications are invited from qualified candidates for admission into the National Diploma (ND) Nursing Programme at CONSA.

Admission Requirements

To be eligible, you must:

• Have a current Joint Admissions and Matriculation Board (JAMB) UTME Registration Number.
• Score a minimum of 100 in the UTME.
• Choose College of Nursing Sciences, Adazi-Nnukwu as your first choice before the Post UTME Screening.

In addition, you need:

• At least five credits in English Language, Mathematics, Biology, Chemistry, and Physics from WAEC, NECO, or NABTEB.
• These credits must be obtained in no more than two sittings.
• You can combine WAEC/WAEC, NECO/NECO, or WAEC/NECO results.
• NABTEB results cannot be combined with WAEC, NECO, or another NABTEB result.

Other requirements include:

• Original WAEC, NECO or NABTEB Certificate or computer printout.
• Upload your O’Level result to the JAMB Portal if you registered UTME with awaiting results.
• A testimonial from the school where you obtained your WAEC/NECO.
• Birth certificate from the National Population Commission, Government Hospital, or a Declaration of Age from the Court.
• A functional personal email.

Post UTME Screening Details

The Post UTME Screening will be held on:

• Date: Wednesday, 25th – Friday, 27th September, 2024
• Venue: College of Nursing Sciences, Adazi-Nnukwu CBT Centre
• Subjects: Use of English, Biology, Chemistry, and Physics

How to Apply

To apply, follow these steps:

1. Pay a non-refundable fee of N15,500 into the school’s bank account:
   • Bank: Uzondu Microfinance Bank
   • Account Name: College of Nursing Sciences, Adazi-Nnukwu
   • Account Number: 1100635153
2. Ensure payment is made in the candidate’s name.
3. Indicate the programme you are applying for (e.g., ND Nursing).
4. Payment can be made via bank, bank app, or POS machine.
5. If paying via bank app or POS, write the candidate’s name and the programme in the narration/remark section.
6. Upload your bank teller or transaction receipt as proof of payment.
7. Bring the proof of payment to the examination hall.
